4.2 Most important symptoms/effects, acute and delayedSYMPTOMS: This compound is a cholinesterase inhibitor. Symptoms from exposure to this compound include anorexia; nausea; vomiting; diarrhea; excessive salivation; pupillary constriction; bronchoconstriction; muscle twitching; convulsions; coma; respiratory failure; pulmonary edema; perspiration; dyspnea; and cyanosis. The effects are cumulative. ACUTE/CHRONIC HAZARDS: This compound may be absorbed readily through the skin. Acute exposure is evidenced by rapid onset of blurred vision and constricted pupils.
4.3 Indication of immediate medical attention and special treatment needed, if necessaryAirway protection. Ensure that a clear airway exists. Intubate the patients and aspirate the secretions with a large-bore suction device if necessary. Administer oxygen by mechanically assisted pulmonary ventilation if respiration is depressed. Improve tissue oxygenation as much as possible before administering atropine, so as to minimize the risk of ventricular fibrillation. One microorganism, Pseudomonas putida, cleaved the thioester bond to form 2-(ethylsulfinyl)ethanethiol, which was subsequently oxidized to bis(2-(ethylsulfinyl)ethyl)disulfide, and 2-(ethylsulfonyl)ethanethiol(1). Another microorganism, a Nocardia sp. produced bis(2-(ethylsulfonyl)ethyl)disulfide, bis[2-(ethylsulfinyl)ethyl]sulfide, and bis(2-(ethylsulfonyl)ethyl)sulfide via oxidation, beta-elimination, and hydrolysis(1).
